What Can Rhinoplasty Treat?

Aesthetic Issues

Many patients are interested in rhinoplasty to improve their appearance and self-confidence. Rhinoplasty surgery can address:

  • A nose that is too large or too wide in proportion to other facial features
  • A sizable bump on the nasal bridge, which may disrupt the facial profile
  • A nasal tip that droops, is too thick, or is enlarged
  • An over-projected or under-projected nasal tip
  • Nostrils that are too large or misshapen
  • Overly thin or short nasal bones
  • An off-center or crooked nose that disturbs the symmetry of the face

Functional Issues

Rhinoplasty isn't just about looks; it can also improve your health. Misshapen or abnormally positioned nasal structures can cause difficulty breathing. When patients have medical conditions such as a deviated septum (misaligned cartilage between the nostrils), it obstructs the patient’s airway. Resolving this issue requires nose surgery. Dr. Lerman has the training and experience to correct the structural problems associated with a misaligned septum.

This 30-year-old male patient came to Dr. Lerman's New York City practice to improve his breathing and the appearance of his nose. The hump on the bridge of the nose was smoothed down and the nasal tip was refined. In addition, Dr. Lerman adjusted the internal structure of the nose to help him breathe properly.

Open Rhinoplasty vs. Closed Rhinoplasty Learn About Your Options

There are two primary approaches to cosmetic nasal surgery:

Closed Rhinoplasty Approach

During a closed rhinoplasty, Dr. Lerman will make a small incision hidden inside the nose. This approach results in absolutely no visible postsurgical scarring because all incisions are made in the interior of the nose. However, this approach has limitations. It can be performed to address obstructed airways, minor nasal tip problems, or a bump on the bridge of the nose.

While the closed approach is ideal for certain cases, Dr. Lerman will recommend the open approach if it better addresses your cosmetic and functional concerns. He will determine the best approach during your consultation at our Manhattan or Westchester County offices.

Open Rhinoplasty Approach

In the open rhinoplasty procedure, Dr. Lerman will make a small incision across the columella, the small piece of tissue between the nostrils. This approach offers better access to the nasal bones and cartilage so more significant aesthetic changes can be made. The soft tissues that cover the nose are exposed and gently raised so Dr. Lerman can reshape the nose structure. In addition, this approach allows Dr. Lerman to build up the nose.

Using cartilage harvested from other areas of the body, such as the septum or ear, Dr. Lerman can augment abnormal nasal structures and straighten them to improve symmetry and respiration.

We're Proud to Have Made a Difference For This Rhinoplasty Patient

"I had a unique and excellent surgical experience with Dr. Lerman when my ENT diagnosed that my breathing problem was due to a collapse of my nasal passages.  SO my procedure was not aesthetic; however, now I can both breathe and my "new nose" looks great!  You won't find a more highly skilled, professional, clear communicator than Dr. Lerman.  His office staff is equally supportive, responsive and professional." We Bring Your Ideas to Life Watch Your Face Evolve As You Try on Different Noses
Using Our Advanced 3D Imagery

Our sophisticated 3D imaging lets you discuss your preferences for nose surgery
with Dr. Lerman and then see the results in real time during the consultation.

Compare How Different Noses
Look on Your Face

Our Crisalix 3D imaging system overcomes a major communication hurdle in plastic surgery. Patients have long struggled to use words to "paint" an accurate picture of their wishes.

They often resort to flipping through before-and-after pictures of other patients to better explain their thoughts. But a picture of a nose on someone else's face can't show how it will look on your own.

The Crisalix 3D imaging allows us to create a 3D image of your face and then alter the nose to try on different shapes to see how they work with your profile. With your face displayed on a shared screen and adjusted as you talk, communication is no longer limited to individual imaginations.

You'll also have access to the simulated results after your consultation so you have time to reflect on the changes and share them with family and friends before making a final selection.

You will be able to truly collaborate with Dr. Lerman, making suggestions and seeing the changes on your face as you talk.

Our advanced imaging technology:
  • ▶ Allows you to discuss your preferences with Dr. Lerman and then see the results in real time on a virtual image of your face
  • ▶ Lets you try on different looks so you can compare the different versions. You can adjust the width, the angle of the nasal tip, and other features of your nose
  • ▶ Helps you see the changes from every angle for a more comprehensive digital approximation

Rhinoplasty Surgery Candidates

You can learn more about your candidacy during a comprehensive consultation and examination with our Manhattan facial plastic surgeon. Good candidates should:

  • Have stopped growing so the nose is fully developed
  • Not smoke
  • Be in good physical health
  • Have specific and realistic treatment goals

The Rhinoplasty Procedure Step-By-Step

Everybody’s nose is unique, and each rhinoplasty procedure is performed according to your personalized plan. In general, most patients can expect:

Your specific surgery may involve augmenting, reducing, and refining certain areas of the nose, such as the cartilage, to achieve the results you desire.
Your specific surgery may involve augmenting, reducing, and refining certain areas of the nose, such as the cartilage, to achieve the results you desire.

Anesthesia

After administering anesthesia to ensure your complete comfort throughout the duration of your procedure, your board-certified surgeon will begin your surgery.

Incisions

Based on our recommendations and the treatment plan that was developed during your consultation at our New York City offices, Dr. Lerman will create the necessary incisions. These incisions will be as small and discreet as possible while still allowing Dr. Lerman to achieve the best possible results.

Nasal Reshaping

Next, Dr. Lerman will make any necessary alterations to help you achieve your aesthetic goals. This may involve augmenting, reducing, and refining certain areas of the nose, such as the bridge, tip, nostrils, cartilage, or septum.

Sutures

Non-dissolvable or dissolvable sutures may be used on the inside or outside of the nose to close the incisions.

Splint

A splint or internal packing may be placed to aid in healing and contouring after your nose job. Dr. Lerman will instruct you on the proper use and wear time for your splint, so there's no confusion. He will also likely remove it himself on a follow-up visit.

Unsatisfied With Your Past Nose Job? Consider Revision Rhinoplasty

If you are unhappy with the results of a previous surgery, have experienced healing issues, or have developed scar tissue, you may wish to undergo revision rhinoplasty. This procedure tends to be even more complex than primary rhinoplasty, so it is important to choose a surgeon with extensive training, experience, and credentials. During your consultation at our Manhattan or Westchester County offices, we can develop a treatment plan for revision rhinoplasty to help you finally achieve the results you desire through a nose job.

When Will I See Results from Rhinoplasty?

Your nasal results will become more apparent as swelling subsides, with most of the healing taking place within the first few months. However, it is important to note that it can take a full year to completely heal.

Have You Considered
Nonsurgical Rhinoplasty?

Dr. Lerman shaking hands with a patient during a consultation

Are you unhappy with the appearance of your nose but not yet ready to undergo nasal surgery? Nonsurgical rhinoplasty using injectable fillers is another option.

Often called a liquid nose job, dermal fillers can be used to change the shape of your nose in a procedure that just takes a few minutes.

The filler, usually hyaluronic acid such as Restylane® and JUVÉDERM®, is placed under the skin to create more volume or smoother lines. It can last anywhere from four months to three years, depending on your skin, the filler, and your desired nasal results. Dr. Lerman can carefully target the right areas to:

  • Add volume to the nose
  • Smooth out small bumps in the nose
  • Lift the tip of your nose
  • Make the tip of the nose more prominent

There are two scenarios when injectables would not produce the desired result. Injectables cannot make a nose smaller and, if you want a dramatic change, Dr. Lerman may recommend surgical rhinoplasty. Liquid rhinoplasty is typically only suitable for subtle, yet noticeable, nasal results.

FAQ About Rhinoplasty

We like to think ahead by anticipating and answering common questions about rhinoplasty surgery, like:

Q. Can rhinoplasty make literally any change to my nose possible?

A. No–and any rhinoplasty surgeon who promises that it can is giving you false expectations. Our plastic surgeon takes into account your nasal passages, cartilage, muscles, and other unique factors to determine what is safe and realistic for you. Depending on your unique nose and underlying structures, he can plan how to reshape your nose while ensuring you remain healthy and safe.

Q. Can rhinoplasty treat aesthetic and functional concerns at the same time?

A. Absolutely, as long as you choose a surgeon with the ability to address both needs, like Dr. Lerman. That means you'll only need one surgery to look and feel your best.

Q. Is it common for men to seek rhinoplasty surgery?

A. Yes, it is. According to the American Society of Plastic Surgeons, 44,503 patients sought rhinoplasty in 2022. Of that number, 6,213, or 14%, were male. You can also rest assured that our doctor will keep your goals in mind to create a strong nose that complements your masculine facial structure.

Q. How long does nose surgery take?

A. The exact length will depend on your goals and your nasal structure. Generally speaking, though, it will likely be completed in two to three hours by our Manhattan doctor.
