This 30-year-old male patient came to Dr. Lerman's New York City practice to improve his breathing and the appearance of his nose. Rhinoplasty (surgical refinement of the nose) can enhance your profile. The surgery can refine the bridge of your nose, the symmetry of your nose, the tip of your nose, and more. Candidates for rhinoplasty include people who want to enhance their appearance, have breathing difficulties, or have suffered facial trauma.

What Can Rhinoplasty Treat?

Aesthetic Issues

Many patients are interested in rhinoplasty to improve their appearance and self-confidence. Rhinoplasty surgery can address:

  • A nose that is too large or too wide in proportion to other facial features 
  • A sizable bump on the nasal bridge, which may disrupt the facial profile
  • A nasal tip that droops, is too thick, or enlarged
  • An over-projected or under-projected nasal tip
  • Nostrils that are too large or misshapen 
  • Overly thin or short nasal bones
  • An off-center or crooked nose that disturbs the symmetry of the face

Functional Issues

Misshapen or abnormally positioned nasal structures can cause difficulty breathing. When patients have medical conditions such as a deviated septum (misaligned cartilage between the nostrils), it obstructs the patient’s airway. Resolving this issue requires nose surgery. Open vs. Closed Rhinoplasty

There are two primary approaches to cosmetic nasal surgery:

Closed Approach

During a closed rhinoplasty, Dr. Lerman will make a small incision hidden inside the nose. This approach results in absolutely no visible postsurgical scarring because all incisions are made in the interior of the nose. However, this approach has limitations. It can be performed to address obstructed airways, minor nasal tip problems, or a bump on the bridge of the nose. While the closed approach is ideal for certain cases, Dr. Lerman will recommend the open approach if it is a better means of addressing your cosmetic and functional concerns. He will determine the best approach during your consultation at our Manhattan or Westchester County offices.

Open Approach

In the open procedure, Dr. Lerman will make a small incision across the columella, the small piece of tissue between the nostrils. This approach offers better access to the nasal bones and cartilage so more significant aesthetic changes can be made. The soft tissues that cover the nose are exposed and gently raised so Dr. Lerman can reshape the nose structure. In addition, this approach allows Dr. Lerman to build up the nose. Using cartilage harvested from other areas of the body, such as the septum or ear, Dr. Lerman can augment abnormal nasal structures and straighten them to improve symmetry and respiration.

Revision Rhinoplasty

If you are unhappy with the results of a previous surgery, have experienced healing issues, or have developed scar tissue, you may wish to undergo revision rhinoplasty. This procedure tends to be even more complex than primary rhinoplasty, so it is important to choose a surgeon with extensive training, experience, and credentials. During your consultation at our Manhattan or Westchester County offices, we can develop a treatment plan to help you finally achieve the results you desire through a nose job.

The Rhinoplasty Procedure Step-By-Step

Everybody’s nose is unique, and each rhinoplasty procedure is performed according to your personalized plan. In general, most patients can expect:


After administering anesthesia to ensure your complete comfort throughout the duration of your procedure, your board-certified surgeon will begin your surgery.


Based on our recommendations and the treatment plan that was developed during your consultation at our Manhattan or Westchester County offices, Dr. Lerman will create the necessary incisions. These incisions will be as small and discreet as possible, while still allowing Dr. Lerman to achieve the best possible results.

Nasal Reshaping

Next, Dr. Lerman will make any necessary alterations to help you achieve your aesthetic goals. This may involve augmenting, reducing, and refining certain areas of the nose, such as the bridge, tip, or nostrils.


Non-dissolvable or dissolvable sutures may be used on the inside or outside of the nose to close the incisions.


A splint or internal packing may be placed to aid in healing and contouring after your nose job.

When Will I See Results from Rhinoplasty?

Your results will become more apparent as swelling subsides, with most of the healing taking place within the first few months. However, it is important to note that it can take a full year to completely heal.
